Paintball Waiver of Liability

I, __________________, assert my desire to participating in paintball activities at {facility}. I understand that paintball involves projectiles in motion, rapidly moving players, and other potential hazards, and that I am engaging in this activity at my own risk.

I acknowledge that the risks include, but are not limited to, damage to clothing or other properties, along with serious and permanent injuries, including: bruising, eye injuries (including blindness), fractures, heat stroke, heart issues, and even death. I am responsible for my own safety, and further recognize that injuries can occur from negligent behavior by myself or others, and that paintball equipment can malfunction as well.

I further assert that I promise to follow the safety precautions as outlined by {facility} staff. Specifically, I agree:

• I have watched the safety video and practice all safety and sportsmanship guidance therein.
• I will wear appropriate clothing to avoid significant injury, including long sleeves and closed-toe shoes.
• I will wear goggles at all times.
• If providing my own paintball gun and ammunition, I will ensure that my marker is chronographed not to exceed {number} feet per second.
• I will avoid bumping into, colliding, or otherwise making contact with objects or persons in a manner that results in physical injury.
• I will not argue with referees, and I understand that all calls are final. I will not ask for unnecessary “paint checks.” I also understand that I am not to use referees as “cover,” and will avoid them when in my line of fire.

I agree to hold {facility} harmless in the event of property loss or personal injury, and by signing this document I assert that I will not seek claims or damages against {facility}, nor will I participate in any class action suits.

In the event of injury, I understand that I will be solely responsible for the cost of treatment. Moreover, I authorize {facility} staff to render emergency first aid. I further assert that I am at least 18 years of age and have no health conditions that would be exacerbated by playing paintball.

By signing below, on behalf of myself and my estate and heirs, I indemnify {facility}, as well as their equipment suppliers and manufacturers, from and any all liabilities, expenses, and damages.
