Waiver For Jump Park Or Trampoline

Trampoline Jumper Release of Liability

By jumping at {Jumping Facility}, I hereby accept any risks involved in participation, accept all liability, and will hold harmless {Jumping Facility} for any damages to my property or injuries to my person. This release of liability includes, but is not limited to, damages or injuries incurred due to negligence, carelessness, or defective equipment.

I agree to abide by the precautions and safety instructions put into place by {Jumping Facility}. I further acknowledge that I and any minors in my care have watched the safety video. I understand that risks of trampoline use cannot be eliminated due to the nature of the activity and include, but are not limited to:

• Bumping into, colliding, or otherwise making contact with objects or persons in a manner that results in physical injury.
• Jumping or landing in a manner that causes injury to myself, not limited to: broken teeth, eye injury, bruising, sprains, joint dislocation, bone breakage, full or partial paralysis, or death.
• Being struck by falling objects.
• Experiencing anxiety, exhaustion, or other emotional and physical responses to the activity of jumping.

I agree to mitigate the risk of damages or injury by following the safety rules as follows:

• Emptying my pockets of all items and carrying nothing upon my person. (Includes but is not limited to toys and balloons.)
• No food, drinks, or gum allowed in the jumping area.
• Wearing clothing appropriate for jumping, wearing only the socks issued by {Jumping Facility}, and removing all jewelry. If eyeglasses are worn, they must be properly secured.
• No “double jumping” (with more than one person per trampoline, causing a rebound effect).
• No racing, wrestling, or lying down on the court.
• No running into walls with the intention of flipping, somersaulting, or otherwise performing acrobatics upon the trampoline surface. (Flipping and other aerial skills are only permitted {location/situation/skill level}. No flipping over the pads between trampolines.
• Jumping with or near jumpers near my own age or size.
• Staying an arm’s length away from other jumpers at all times.
• Jumping only in the center of the trampoline(s).
• Knowing my abilities and taking other common-sense precautions not necessarily enumerated in this waiver or by {Jumping Facility} staff.

By signing below, I agree to waive liability for damages or injuries. Specifically, I will be liable for any and all medical costs, attorney fees, and other costs. I also assert that I am in sound physical condition for jumping, and am not under the influence of alcohol or drugs that could impair my ability to participate safely.

I further agree that this waiver will remain on file and apply to all future visits to {Jumping Facility}.
