Provision for Simultaneous Death

I, {Name}, of {Address}, born {date}, and being of sound mental health and full mental capacity, do instate the following provision to my will regarding the division and dispersal of my estate:

I hereby declare that any named beneficiary who dies simultaneously with me or dies up to {number} days after my death will not receive any inheritance, bequeathals, or monetary gain assigned to them from my last will and testament. Any benefits assigned to them will be passed to the alternative beneficiary named. If there is no beneficiary named, the benefits assigned will be given to {Name/Organization}.

Should my spouse die simultaneously with me or up to {number} days after me, the Guardian Provision and Provision of Trust for our minor children will go into effect.

I, {Name}, hereby declare this provision to be an addition or change to my will. All other aspects of my will are to remain intact.
