By participating in the snorkel or SCUBA excursion through {Company Name}, I hereby accept any and all risks involved and will hold harmless this business as well as its employees and instructors for any injuries to my person while in the water, on the boat, or {other location}. This release of liability includes, but is not limited to, damages or injuries due to diving, swimming, and boating.

I understand that this activity has inherent risks, including:

• Drowning.
• Equipment failure.
• Cuts or other injuries related to coral or other marine life.
• Attacks by sharks or other sea life.
• Boat accidents.
• Motion sickness.

I agree to:

• Use equipment only as intended.
• Listen attentively during briefing and instructions.
• Follow the "buddy system" and never snorkel or dive alone.
• Respect and refrain from contact with all ocean life, reefs, and other natural habitat.

By signing below, I agree to waive liability for damages or injuries. Specifically, I will be liable for any and all medical expenses, attorney fees, and other costs. I also assert that I am of sound physical fitness for snorkeling or diving and am free of heart ailments, high blood pressure, or respiratory problems. Moreover, I do not suffer from motion sickness, claustrophobia, balance or equilibrium issues, or any other illnesses or health conditions that could impact my ability to safely participate in this activity. {For SCUBA, include clause regarding any required certification or lessons}. I also affirm that I am not under the influence of alcohol or any drugs that could impair my judgment while on the boat or in the water.
