By skateboarding, in-line skating, BMX bicycle riding, or engaging in other allowed activities at {Skate Park Name}, I hereby accept any and all risks involved and will hold harmless this {business/city park/etc.} and its management and/or attendants for any injuries to my person.

I understand that I am skating at my own risk, and potential dangers and injuries include but are not limited to:

• Injuries related to falls and crashes, including broken bones, concussions, disability, and death.
• Colliding with another person.
• Equipment or track failure resulting in falls and injuries.
• Property damage or loss.

I agree to the following rules:

• Helmets are required to be worn at all times while on the skating surface. Pads are highly recommended.
• Look carefully for other skaters in your path, especially on ramps, curbs, half-pipes, and obstacles. Be especially aware of younger/inexperienced skaters.
• No fighting or aggressive behavior.
• No drugs, alcohol, or smoking/vaping.
• Skaters under {age} must be supervised by an adult.

By signing below, I agree to waive liability for damages or injuries. Specifically, I will be liable for any and all medical expenses, attorney fees, and other costs. I also assert that I am of sound physical fitness for skating.
