Research Study Agreement and Waiver

_______________________ (hereafter “Participant”) and _______________________ (hereafter “Researcher”), hereby agree to the following rules and restrictions for participation in the _____________________ study (hereafter “Study”):

The Study will take place from {date} to {date}, with a total of {number} {time-period} sessions. Rescheduling in the event of a conflict may be possible with {amount of time} notice. The Participant will be paid {amount} per {hour/session/whole study}. The Participant {must/is not required to} complete all sessions to be eligible for payment.

The Participant verifies and affirms that they do not have {qualities that would exclude them from participating or would qualify as a conflict of interest}.

The Study is being run for the purposes of {purpose}. The Participant understands that for the duration of the study, they will be asked to do the following things: {requirements}.

The Participant consents to being {filmed/recorded/documented} during the Study. The Participant understands that the information will be used for {article/publication/study} and shared with the public with the following modifications for privacy: {modifications}. The Participant waives the right to privacy and authorizes the release of the personal information recorded in this Study, as follows: {information}.

The Participant understands and agrees to participate in the following elements of the Study that might cause some risks, discomfort, or physical or psychological harm: {elements, harm}.

The Participant understands that the benefit of this study would be {benefit} and would benefit {the Participant/society/demographic}. Benefits to the Participant are not guaranteed by the Researcher.

The Participant understands that they may withdraw from the Study at any time.
