Release for Use of Likeness

I swear and attest that I grant this right exclusively to {Company} and have not granted the same permission to any other person or organization.

I understand that from this point forward {Company} may use my likeness for any purpose, including but not limited to advertising, promotion, film, posters, photographs, radio, and other formats. My likeness may be copyrighted or used in conjunction with other material. {Company} may alter, edit, color, dub, and ascribe fictitious names to this material as they desire. I waive all rights to view or approve any use or alterations to this material.

In return for my services, I have received {exchange} and will also receive {future exchange conditions}.
