Provision for Cremation

I, {Name}, of {Address}, born {date}, and being of sound mental health and full mental capacity, do instate the following provision to my will regarding the disposal of my remains:

In the event of my death, I hereby direct that my body shall be cremated immediately, without autopsy, unless the circumstances of my death are unnatural or suspicious.

It is my further desire that my remains, once cremated, shall be placed in {type of container or urn}, with the following information stated clearly: {Name, dates, etc.}. I would like the container to be present for a funeral service at {location}, officiated by {Name} if possible.

Once the service has concluded, my remains may be disposed of however {Name} sees fit. If {Name} predeceases me, I entrust my executor with proper disposal.

I, {Name}, hereby declare this provision to be an addition or change to my will. Any previous instructions for the disposal of my remains shall be considered nullified. All other aspects of my will are to remain intact.
