Provision for Burial

I, {Name}, of {Address}, born {date}, and being of sound mental health and full mental capacity, do instate the following provision to my will regarding the disposal of my remains:

In the event of my death, I hereby direct that my body shall be sent to be embalmed at {Establishment}. No autopsy shall be performed unless my death occurs under unnatural and suspicious circumstances.
