Provision for Pet

I, {Name}, of {Address}, born {date}, and being of sound mental health and full mental capacity, do instate the following provision to my will regarding the division and dispersal of my estate:

It is my wish that my beloved {animal breed}, {Pet Name}, be given to {Name}. If {Name} is unable to care for {Pet Name}, {Pet Name} shall go to {Other Name}.

In addition to {Pet Name}, {Name} will be given {amount of money} for the sole purpose of caring for {Pet Name} and providing the food, shelter, health care, and comfort {he/she} needs for the rest of {his/her} days. I expect {Pet Name} to be treated with the love and care I have provided, and if {Name} is unable to do so, I request that {he/she} find a suitable and loving home for {Pet Name}.

I hereby direct {Name} to provide the following necessary services for {Pet Name}:

• {Frequency} checkups at a veterinary clinic.
• Proper, timely and consistent administration of all necessary medications and vaccinations.
• {Type of food}, at least {amount} {frequency}, but no more than {amount} {frequency}.
• A warm, comfortable place to sleep {indoors/outdoors}.
• Proper hygiene surrounding cleanliness, waste disposal, and habitat.

I, {Name}, hereby declare this provision to be an addition or change to my will. All other aspects of my will are to remain intact.
