Permission to Reuse Music

PERMISSION TO REUSE MUSIC

This agreement is hereby made between {Contractor}, {Musician/Lyricist/Composer} and {Licensing Studio}.

The Contractor is hereby granted permission to reuse the following music/lyrics:

{List of Song Titles and/or lyrics. If sheet music is being used, please indicate}
or
{Description of music title selection from music library, as well as the dates the Contractor is allowed access}

The Contractor may use the aforementioned music in the following capacity:

• Educational use only: the Contractor may present the material only to students who are registered for {Class} during the {term} term at {School Name}. The material may be presented in {audio/visual} format, and recordings {will/will not} be available to students for the duration of the class.
• Soundtrack: the Contractor will play {Song Title} from {time} to {time} in the soundtrack for the {description of project} project. {Title, director, studio, minutes, context, description}. The project will be released in the following formats: {formats, access, price per viewing}. The song {will/will not} appear on a published soundtrack made available after the release of the project.
• Performance: the Contractor may perform the song at {venue(s)} on {date(s)}. No recordings may be released or distributed for commercial purposes.
• Cover: the Contractor has permission to modify the song in the following ways: {modifications} and perform the song in {performance capacity}.

In exchange for these services, the Contractor will pay {amount, royalties, etc.}. If the Contractor engages in use of the copyrighted materials outside of the conditions set down in this agreement, {he/she} shall be subject to {penalties}.
