Permission To Use Copyright Materials

PERMISSION TO USE COPYRIGHT MATERIAL

This agreement is hereby made between {Licensor} and {Licensee} regarding the usage of copyright material:

In that the Licensor has the full and legal rights over {Material}, and that the Licensee is seeking non-exclusive rights to {perform/reprint} said material.

The copyright material shall be {performed X number of times on dates/reprinted in X number of copies of Book, Author, Publisher, Date}.

The Licensee agrees to use the copyright material solely in the context outlined above and to include proper attribution to the original creator and the Licensor, as follows: "{This material was created by Creator and published by Publisher, Date, Licensor}."

The permission for reuse is for {number} uses only. Should subsequent reprinting or performances be desired, this contract shall be amended by both parties, in writing.

In exchange for these services, the Licensee will pay {amount, royalties, etc.}. If the Licensee engages in use of the copyrighted materials outside of the conditions set down in this agreement, {he/she} shall be subject to {penalties}.
