Part Time Child Care Contract

This contract covers the arrangement made between {Care Provider} and {Parent} concerning part time care of {Child}. This part time care will occur at {Name of Daycare, house, or other location} and will begin on {Date}.

By signing this agreement, {Parent} agrees to all conditions set forth below, as well as all care guidelines outlined in writing by {Care Provider}. These guidelines cover all care procedures, and {Parent} certifies that these guidelines have been provided in writing. Both this contract and a copy of the guidelines must be signed by {Parent} before care can begin.

{Parent} accepts the rate of {rate} per {day/week/month} and will pay this rate in advance on {regular payment due date}. Payment must be received before care can begin.

Part time care of {Child} will occur during the hours of {list usual hours} and will not extend for more than {maximum hours}. Additional care may be provided at an additional cost.

The duties of {Care Provider} are limited to those included in this document and the written guidelines. Alterations to these guidelines or the contract must be documented in writing and filed, including minor exceptions. In case of rate increases or other changes to the contract, {Parent} will be given {number of weeks} notice, at which time a new agreement must be signed. A child who does not have a contract on file cannot receive care.
