Music Lesson Contract

_______________________ (hereafter "Instructor") and ___________________________ (hereafter "Student") hereby agree to the following contract concerning lessons in {instrument}:

The Student will have {daily, weekly, etc.} lessons at {location}. Lessons will begin on {date} and end on {date, if desired}.

Each lesson will last {amount of time} and cost {amount}. Lessons may be extended to {amount of time} at {cost} with the Teacher's approval. Payment can be made upon services delivered. Payment made in advance for the semester will be granted a discount of {amount}, for a total of {amount}.

The Student will provide his/her own instrument for practice and performance. The Teacher shall provide an additional instrument for instructional purposes. The Student will buy the required sheet music and equipment at the Teacher's recommendation.

The Teacher will provide the opportunity for {number} recitals, which will fall roughly at {times/seasons}. The Student will perform {solo/alongside other students}.

The Student is responsible for his/her own attendance. If the Student cannot attend a lesson, he/she must notify the Teacher at least {amount of time} in advance. If there is a medical emergency, a written note from a doctor can be substituted for advance notice, and the student will not be charged. Otherwise, the Student will still be charged {amount} for the missed lesson.
