Lab Use Agreement

_____________________________ (hereafter "Institution") and _______________________ (hereafter "User") hereby agree to the following use agreement regarding the property at ______________________ (hereafter "Lab"):

The Institution agrees to allow the User to occupy the Lab {from date to date or on certain days} at {time(s)}, strictly for the purposes of {purpose}.

The User agrees to pay {amount} for use of the Lab, including a {monetary amount} deposit.

The User agrees to obtain liability insurance with a minimum occurrence limit of {amount} and citing the Institution as an additional insured. The Institution must receive the certificate of liability insurance no later than {number} days before the User is permitted to use the Lab.

The Institution will provide {equipment/chemicals/funding}. The User will be responsible for all other prerequisites to conduct {his/her} study or experiments.

Processes that may negatively affect the Lab's equipment must be approved by the Institution in writing prior to implementation.

The User will be charged for any cost made to the Lab during {his/her} occupation, including set-up, cleaning, maintenance, repair and disposal of waste or chemicals. The User will be held responsible for any penalties, damages or theft incurred while using the Lab.

The User will check for compliance with the Institution before introducing any new chemicals into the Lab.

A representative of the Institution will be present at all times of the User's occupation to ensure safety and compliance.

The User shall ensure that all egresses in the Lab are accessible and free from obstruction at all times.

The User agrees to abide by all local, state and federal laws while using the Lab.

The User understands that work in this laboratory can result in exposure to dangerous chemicals. The User agrees that {he/she} and all affiliates, students and attendees will wear proper safety gear at all times, including protective eyewear and gloves. The User agrees to abide by the Institution's lab safety protocol, including {Protocol}.

The User agrees to indemnify and hold harmless the Institution and all of {his/her} affiliates, employees and representatives for any loss, injury, damage or theft that occurs at the Lab.

The User is prohibited from occupying {area(s)} at the Lab.

The User shall make no alterations to the lab without prior written approval from the Institution.

The Institution will provide {keys/equipment} for the User's use only.

The Institution shall have no claim over the User's intellectual property, research results or preliminary data. The User is under no obligation to share research information with the Institution.

The User shall not use the Institution's name, image, or likeness in promotional materials nor treat the Institution as an affiliate or means of endorsement.

Should the User violate the terms and conditions of the use of the Lab, {he/she} is subject to immediate eviction of the Lab and dissolution of this contract.

Either party may withdraw from this agreement with {amount of time} notice.
