Household Isolation Agreement

_______________________ (hereafter "Party 1"), _______________________ (hereafter "Party 2"), _______________________ (hereafter "Party 3"), _______________________ (hereafter "Party 4"), and _______________________ (hereafter "Party 5") (collectively, "The Household") hereby agree to the following:

Due to unforeseen and uncontrollable health risks and danger to the vulnerable within the community, the Household agrees to take every precaution to limit exposure and protect each other and others in the following ways:

Each party agrees not to come into close contact with any parties outside of the household (e.g. sharing indoor spaces, sharing food or surfaces, standing within six feet of another person).

Each party agrees to immediately limit unnecessary outings, errands, and appointments. Outings are limited to {frequency} grocery store runs, bank and post office visits, and outdoor exercise that observes the six-foot radius limit around others.

Each party agrees to cough or sneeze into an elbow or a tissue and to wash hands immediately after doing so.

Each party agrees to wash hands before making food, before leaving the house, after entering the house, after touching one's face, and after using the bathroom.

Each party agrees to wash hands each time for at least twenty seconds using hot water and soap, and to practice good handwashing technique.

Party {X} agrees to make {frequency} trips to the grocery store so as to limit contact.

The Household agrees to divide chores and cooking fairly and equally during this time of prolonged indoor isolation.

Parties that are working from home agree to create a schedule for when they are working. Noise levels of {level} will be observed during this time, and working parties are not to be disturbed except in the case of emergency.

Quiet times start at {time} and last until {time} each night.

Any party experiencing {symptoms} agrees to immediately notify the Household and self-quarantine in a private room separate from other household members.
