By visiting {Haunted House}, I hereby accept any risks involved in participation, accept all liability, and will hold harmless {Company/Organization} for any damages or injuries incurred therein. This release of liability includes, but is not limited to, damages or injuries incurred due to negligence, carelessness, property, or defective equipment.

I agree to abide by the precautions and safety instructions put into place by {Company/Organization}. I understand that risks of visiting the haunted house include, but are not limited to:

• Bumping into, colliding, or otherwise making contact with objects or persons.
• Tripping, falling, stumbling, etc.
• Being struck by falling objects.
• Vision impairment due to fog machine, dry ice, etc.
• Experiencing anxiety, stress, claustrophobia, extreme fright, fear of the dark, nightmares, or other heightened emotions.

By signing below, I agree that I waive liability for damages or injuries. I also assert that I am in sound mental and physical condition to visit this haunted house, and am not under the influence of alcohol or drugs that could impair my ability to participate safely.
