By participating in {class/bootcamp/etc.} run by {Business Name}, I hereby accept the risks involved and will hold harmless this business, as well as its instructors and facility, for any injuries to my person.

I understand that exercising and other fitness-related activities involve inherent risks, including, but not limited to:

• Injuries related to physical exercise.
• Colliding with another person.
• Gym or other equipment failure.
• Medical emergencies related to physical exertion.
• Exhaustion, dehydration, and other effects not listed herein.

By signing below, I agree to waive liability for damages or injuries. Specifically, I will be liable for any and all medical expenses, attorney fees, and other costs. I also assert that I am of sound physical fitness and am free of heart conditions, asthma, joint issues, or any other health conditions that could impact my ability to safely participate in exercise courses involving low- to high-impact physical exertion.
