Family Phone Plan Agreement

_______________________ (hereafter "Party 1"), _______________________ (hereafter "Party 2"), _______________________ (hereafter "Party 3"), _______________________ (hereafter "Party 4"), and _______________________ (hereafter "Party 5") (collectively, "The Household") hereby agree to the following:

The Household agrees to get a {plan type} plan from {Carrier}. This plan will include {number} lines for {phone types} with {amount} of data split between all parties for {amount total}. All parties commit to participating in this plan for its entire duration of {number} years.

Party {X} will be the main contact on the plan. {He/she} will make monthly payments to the carrier; check the data usage each month to make sure it is not being exceeded; collect payments from the Household; and keep track of account information such as username, password, and security questions.

All parties over the age of {age} will be responsible for the payment of their phone (either in one lump sum upon purchase or in monthly installments) until the phone is paid off in full, even if it is lost, stolen, or broken. All parties over the age of {age} will be responsible for paying Party {X} {amount} per month for the phone/data plan. The phone and data payments may be combined into one payment to Party {X} each month, no later than {day}.

Each party is permitted the use of {amount} of data each month. Any party who exceeds their data use and causes an overage charge to the account will be solely responsible for paying said fine in addition to their normal monthly payments. This includes parties under the age of {age}.

Each party will be responsible for the safety and security of their own devices.
