Drug Abstinence Contract

This contract defines and establishes an agreement between {Child's Name}, hereafter known as Child and {Parents' Names}, hereafter known as Parents, concerning the Child's abstinence from drugs, alcohol and tobacco products. By signing the contract, the Child has read, understood and agreed to the following requirements:

• Child will remain completely drug free and refuse offers of illegal substances.
• Child will abstain from imbibing in alcohol or using tobacco products until of legal age and until Child is residing in {his/her} own place of residence.
• Child will leave any party or gathering where others are participating in underage drinking or use of illegal substances.
• Child knows the Emergency Contacts and numbers to call in order to remove {him/her}self from a dangerous or uncomfortable situation. The names and numbers are:
