Covid Employee Agreement

COVID-19 Employee Contract and Waiver of Liability

This agreement is made between {Employer} (hereafter "The Employer") and {Employee} (hereafter "The Employee") regarding safety measures and liability and is in effect from {date} until the community is no longer at risk for contraction of COVID-19, as defined and specified by the Centers for Disease Control (CDC).

The Employer agrees to provide the following safety precautions while operating during the pandemic:

• No more than {number} people in the building at a time.
• Employees will be spaced {number} feet apart at all times.
• Employees will be given {masks, gloves, hand sanitizer} if they do not provide their own.
• Surfaces will be disinfected every {amount of time}.
• Ventilation will be modified to {settings}.
• Employees in the following positions will work from home and be provided with {materials}: {Positions}.
• Employer will enforce mask-wearing and physical distancing of employees as well as clients and customers in the building at all times.

The Employee agrees to abide by the following requirements:

• Wear a mask over their nose and mouth at all times while in the building.
• Wash hands or use hand sanitizer after touching face or shared surfaces, blowing nose, coughing, sneezing, or using the bathroom.
• Change personal protective equipment (PPE) after {encounters}.

The Employee agrees that while these precautions are in place and enforced, they release the Employer from all liability for the Employee contracting COVID-19, as well as any financial responsibility for medical attention the Employee might require. The Employee understands that even with the precautions in place, there is still a risk of contracting COVID-19 at the workplace.

The Employer agrees that if the Employee contracts COVID-19, the Employer will:

• Provide access for COVID-19 testing.
• Provide 10 days sick leave for the Employee from date of diagnosis or symptoms.
• Notify all employees of exposure risk if someone is diagnosed with COVID-19.
