Consent to Polygraph

By signing this contract, {Employee Name}, hereafter referred to as "Employee," consents to take a polygraph (or lie detector) test at the behest of {Employer}, hereafter referred to as "Employer." The Employee agrees to and understands the following information:

• The commencement or continuation of employment of the Employee will in no way be affected by {his/her} agreement or refusal to take the polygraph test.
• The Employee has not be forced, threatened or coerced in any way into taking the test.
• The Employee voluntarily submits to taking the test.

The test will take place on {date} at {time} in {location}. The results of the polygraph test will be sent to {Company} at {location} for processing. The Employee will be given a copy of the results for {his/her} personal records.
