_______________________ (hereafter "Landlord") and ___________________________ (hereafter "Tenant") both agree to the terms and stipulations with regard to canceling the lease on the following property:

The Tenant leased the aforementioned property on {MM/DD/YYYY} and signed a contract extending that lease to {MM/DD/YYYY} at midnight. However, both Landlord and Tenant agree to dissolve the contract under the following conditions:
The Tenant will {forfeit their deposit, pay a fee of $XX, pay rent through MM/DD/YY, etc.}.
The Tenant will not be required to find a replacement tenant.
The Tenant agrees to be completely moved out of the property (including return of all keys, laundry cards, and borrowed equipment) no later than {MM/DD/YY}at {XX:XX AM/PM}.
The Tenant will leave a forwarding address with the Landlord before departure. The Landlord agrees to forward the security deposit (minus cleaning/repair fees) no later than {MM/DD/YY}.
