I, the undersigned, agree to be videotaped, recorded, and/or photographed for the purpose of ________________________________. I grant this release to _____________________________ as well as any successor, assign or personnel working on this project. I understand that this release does not guarantee me screen or audio time, but that it may extend to production, advertisements, reproductions, and simulations of my image.

I waive the right to view and approve any finished copies. I understand that the finished form will be published in multiple formats and languages. I grant the right of the aforementioned organization to distribute and sell any copies of the final product. I understand that I have been compensated for my time and performance, and that I will receive no additional payments or royalties. I verify that I am at least 18 years of age.
