Pet Therapy Agreement

This agreement is made on {date} between {Client Name}, hereafter “Client,” and {Pet Therapist Name}, hereafter “Therapist,” concerning the following pet:

{Pet Name}, {breed, color, weight, age} (hereafter “The Pet”)

The Client agrees to the following conditions:

• To follow the therapist’s instructions without modification regarding treatment, until such time as the relationship ceases.
• To pay {amount} per session for a total of {number} {amount of time} {frequency} sessions with the Therapist.
• To give at least {amount of time} notice in case of cancellation, or be subject to a fee of {amount}.

The Therapist will provide:

• Professional pet therapy, as certified by {Certification}, following the principles of {approach/technique}. The scope of services includes {elements} but not {elements}.
• {Number} sessions with the Pet and the Client on an {amount of time} basis.
• Insights and techniques to help the Client to interact with the Pet in order to produce the best results. Therapist may also refer Client to a licensed veterinarian for potential evaluation for medication, if warranted.
• Confidentiality regarding Pet, Client, and associated parties, with the exception being any case in which the Pet is deemed to be in danger.

The Therapist cannot guarantee results in individual pets.

The Therapist reserves the right to discontinue training if the Pet is considered a threat to people or other animals, or if the Therapist deems the session to cause undue anxiety or other ill effects to the Pet and/or Client. The Client will be responsible for any damages of injuries caused by the Pet during therapy sessions.
