The parties named above agree that {Pet Owner Name} (hereafter “Owner”) will be boarding {Pet Name} (hereafter “Pet”) at {Boarding Facility Name} (hereafter “Facility”).

1. Boarding services shall be provided at the facility address noted above, commencing on {date} and ending on {date}.

2. Pet Owner shall pay to Facility the sum of {amount} per {day/other}, with {amount} being due at the time of Pet’s drop-off and {amount} due upon pickup. If Pet is not picked up at the agreed-upon time, {policy for various scenarios}.

3. Owner shall provide to Facility proof of Pet’s up-to-date vaccinations, including: {vaccinations}. Also, {flea/tick prevention and other measures}. Moreover, Owner affirms that {details about dog or cat temperament or other expectations}.

4. Supplies including {food, bedding, litter, etc.} shall be provided by {Owner/Facility}. Pet eats {food brand, type, amount, etc.}.

5. Owner hereby acknowledges and agrees that Facility shall not be liable for any disease, infection, injury, death, theft, or other negative effects suffered by Pet during boarding, unless {negligence is proven, etc.}.

6. If Pet requires emergency medical care, Facility agrees to immediately attempt to contact Owner via the following methods: {methods and contact information}. If Facility is unable to contact Owner within a reasonable period of time, Facility staff shall be empowered to obtain emergency veterinary services deemed to be in the best interests of Pet. Owner shall be liable for the cost of all such emergency medical services.
